It’s a difficult experience for any carer to have to hear that there has been an allegation made against them, or there is significant concerns that may question their ongoing approval. It's difficult when placements of children come to an unplanned end, and often leaves the child and carer with unresolved grief and loss.

How often do we fully analyse what has happened and reflect on what we can all do a little better? How often do we do meaningful Disruption Meetings when children leave the care of a family they thought was their “forever” home? When children may have experienced harm to themselves or others, how thoroughly do we explore contributing factors, learn, and ensure that children move to a well matched alternative family or care provider?

The purpose of Reflective Case Reviews is to identify improvements to be made to safeguard and promote the welfare of children. Reflective Case Reviews help in understanding not only what happened, but also why things happened as they did, which can help to improve our response in the future. By  understanding the impact allegations or placement breakdown can have on the child, the carer or  the agency reflective reviews can consider whether different approaches or actions may have resulted in a different outcome. By analysing reflectively we can make better decisions  about what might need to change. It also helps to  gain an understanding of  whether there are systemic issues, and how policy and practice needs to adapt. This all is critical to self-improvement. 

The review will be thorough, open, transparent, with an opportunity to highlight examples of good practice to show that you are a learning and developing organisation. This promotes excellence and leads to being “outstanding”, showing shared practice learning can become embedded and integral to all we do.
